I have the file system set to private. I'm doing everything as the admin.
Uploading files via "file/add" stores files in the correct folder.
Clicking on the file in the "content/file" overview I see the node view for the file. There is a link in the format "system/files/filename.ext". When clicking on the file link the following happens:

With the file_entity module disabled I see the file. If it's an image I see the image, full screen.
With the file_entity module enabled I get "Access Denied" on every file.

How can the admin get a Access Denied message?
