Notice: Undefined index: und in media_gallery_form_alter()(line 864 of .../media_gallery/media_gallery.module)

Go in, uninstall the module, ftp out and remove it from the directory (or git/drush it out), and reinstall it. When you're done reinstalling it, enable it, and then before you do anything else, run drupal's update script. That fixed my issue.
