Version is 4.6.0.

The books heirarchy (block) is lost when a node is translated. Checking out the db, it seems that the parent of the translated node is the parent of the original language. Shouldn't the translated node first check if the there is a translation of that parent node?

If I have the node A and B where B is the japanese translation of the english node A, node C where C is the child book node of A and node D where D is the japanese translation of C, then you would expect to have a relationship of the following:
A-B
| |
C-D

A and C are in English
B and D are in Japanese

The current situation is
A-B
|\
C D

This can easily be manually fixed. . . but is a pain.

There is another problem with the translation link that is offered on the links of the books. It seems that while it gives the correct node id, the language is prepended as the wrong language. That is, if I have node A and B where A is English and B is Japanese, then while reading A I have a link to B at the bottom of the node. If I follow the link, I will get the correct content in Japanese, but the interface will still be English. (Eg. http://www.jccafe.ca/ja/node/12 for an example).

The first one was user error, sorry about that. You have to switch the interface before the proper parents show up in the drop down.

Yes, you have to click 'preview' after changing the field 'language' to get the correct terms an hierarchy, will update the documentation about this

About the second one, it is like that by design. The idea is that you can occassionally see a different language version of a node without completely switching the interface.

If you want to switch both, content and interface, you have to switch language with the main language's block
