Add settings for omitting providers

Issue Summary

For a customer it was needed to just allow the service for gmail, yahoo and msn. However it was not possible to omit some of the providers.

This patch provides a settings page where you can omit the providers.

If you also want to install the Support for MSN custom passport be sure to install that patch first:

This is a feature that I've added into the D6 version. The D5 version is no longer maintained.
