Allow User Role Selection to be required and renamed

This patch allows an option to make User Role Selection a required form element.

It also allows an option to rename the fieldset that contains the form element for User Role Selection. In other words, your custom named form element doesn't have to be inside a fieldset called "User Role Selection."
