provision_apache module missing (and broken in last known revision)

In the "Clean up extreme file clutter in provision module" commit, 9 days ago, that file was simply removed, whereas i think it should probably have been moved into the web_server directory (?).

Anyways, even back then, it was broken and unparseable. On line 140, the function declaration was missing a "function" keyword. The module is also lacking a proper submit handler. One could probably be added after the patch from issue 225571 gets merged in.
