Adapt legal Module for liability waiver

It would be helpful to have a similar module where I can post a liability waiver, and require it to be checked before a member registers for an event.

Since waivers come in different shapes and sizes, it would be helpful to use 'replacement strings' from the event into the waiver, such as the user's name, the event, the date of the event, etc in the form. This would help the site owner customize the waiver according to their attorney's recommendations.

It would read like "I, (@Firstname @Lastname) sign my life away for very dangerous (@Event_Title) on (@event_date) and release (@site) from any liability, blah, blah, blah. [] <-check box to continue!"

Once the box is checked, the waiver is stored on file indefinitely, and the user is taken to the next step in the registration process (payment, confirmation, etc).
