On the Forward Settings page, the text filled out in the "Forward Instructions" field doesn't seem to appear anywhere. I'm also unable to change the title of the forward form page. No matter what I put in the "Forward Page Title" field, I still get "Forward this page" as the title on the form page.

To update the page title, you must clear the menu cache. The missing instructions has been fixed.
