When adding an inline img in TinyMCE 2.1.3 with the insert mode option "Filter Tag", both HTML Code and the filter tag are inserted.

Which browser are you using? Does the same thing happen in other browsers?

Could you also please try with version 6.x-2.x-dev together with the Wysiwyg module instead of the TinyMCE module. (Wysiwyg uses the TinyMCE editor.)
