I couldn't get the player to display in browsers until I determined that the javascript call is for a player called "mediaplayer.swf" but the actual file is called "player.swf". When I renamed the file everything worked.

You need to use the version of JW FLV Player linked to from the Quickstart guide.
Jeroen has released a new version of the player, but it has a different api and older versions are no longer available. This sort of unprofessional behaviour is typical of Jeroen (he has done it before) and is the primary reason why op_video 5.x-4.x will use Flowplayer instead.
Also, Flowplayer is free software (like Drupal).
