As background on the issue, there are two modes of displaying the Forward form.

Mode 1: Inline expands to fill in the form. This works as expected
Mode 2: Click on email link and go to a new page with the form. This does not work. The first part of the form where you enter some text to describe the form is shown. That is the end of the form. There's no other data rendered.

It turns out this module breaks if the tellafriend module was present at some point in time.
