Sorry to open this back up--I have the same issue. I'm using the og_user_roles module if that might be conflicting.

Using coherent_access in connection with organic groups does not work well. Due to the way Drupal's node_access works, we recommend only using one form of access control on a given node.

I'm assuming you are using OG because you are using the og_user_roles module.

Can you clarify your use case?
