THIS IS NOT READY FOR USE.
After a major rewrite of the module I decided to branch off to a version 2. This version (will have) a lot more features than in version 1 including conditional actions support and more fine grained control over the way fees are applied. Most of the admin interface is in place, but a lot of it doesn't actually do anything yet!
