Hi... Upon reading on the documentation on your module, I am not sure if it provide feature like this, "Provide forms for collection of contact details for interested visitors, based on multiple mailing lists via SimpleNews.module"

collection of contact details: only email
"multiple mailing": yes

Collecting more than email is frequently requested. Some solutions discussed (and realized ?) with Webforms module. Search the issue queue for info. Addition of this functionality should not be expected for 5.x.
