It is the development module. Whenever I select and type in the new term, I got a small popup window with that message. Then the vocabulary field is disabled. (the new term did get created when I check back in that vocabulary, it just disabled the vocabulary field during form input). In the link above, I listed some more error log messages.
It is probably not module problem, because I installed on another test site using WAMP and don't have this problem. At home, I use ApacheTriad (with manual update of PHP to 5.2.8) and it get this problem. I know my manual installation of PHP5.2.8 is not the problem, because I just tried the original ApacheTriad and got the same problem. (had some problem installing WAMP at home, maybe I will try it again later).

But the solution didn't work for me.
I saw the error every time I cleaned the cache. As long as I don't clean cache, the error won't happen.

As in subject. The exact error only shows on my production lighttpd server. There's no error in my localhost using xampp (apache) . wondering anything to do with that?

This bug renders the module useless for me. It will be faster for me to develop custom functionality than it will be to troubleshoot this bug.
