Allow for multiple indexes of one vocabulary

Currently there seems to be no way of having multiple vocabindexes for one vocabulary. An example of this would be having two blocks that use the same vocab, one which uses tree, and another using flat. Maybe index should clone how views ui works, where you just create a new index, set if it's page or block, set what vocab it uses, set output style, and save.
