Cannot access settings for forward

I've downloaded and installed the latest available download and it just doesn't work. I can't access the admin settings and an error occurs when I attempt to click on forward tracking:

User Admin
Location /admin/forward
Message call_user_func_array() [function.call-user-func-array]: First argument is expected to be a valid callback, 'forward_tracking' was given in /home/walkerst/public_html/includes/menu.inc on line 354.

I then tried on posted without debug code in this section and got further but not far enough to have a working module.
