I wonder if this support request is actually a bug report. After reviewing countless posts, I think I am the only one having these questions or issues.

question 1. the radio button question looks good however, is there a way to get the value of the radio button to be submitted (e.g., red) versus the number of the radio, in this case 1=red.

question 2. this is the "file" type and seems interesting (if not dangerous ;-). Anyway, if I were to use it, where does the file go? I uploaded two test files and can't find them. OR does this type not work.

question 3. this is the most frustrating one. I am trying to use the checkbox type - commonly used in surveys. I assume it is a syntax error on my part but can't figure it out. In the selections options box, I put in banana;apple;orange. Should this work? OR maybe this type does not work.

Not sure if you have sorted this out but the issue seems to be with the forms module.
