Alter form for editing and adding image gallery terms

The forms for editing and adding image gallery terms at admin/content/image/add do not get altered to add the menu or URL alias settings. The attached patch fixes this - this patch is actually borrowed from the issue with the same name, but targeted towards drupal 5. I'm not sure whether there is a solution for this in the current code, but if it is, it doesn't work for me - this patch does.
