Remove redundant description when adding a question

When you add a new question the user is presented with redundant information, see attached screenshot.

this goes for all question types, so the user are presented with a title first and a similar description right after it:

Create Long answer question
Create a new Long answer question.

Create Matching
Create a new Matching.

Create Multiple choice question
Create a new Multiple choice question.

etc..

note that this is *not* the "Description" field for the content type (which actually could be useful).
