Issue Summary
How do I create a custom subscription page? I mean I need a page that has some text about the newsletter and then the e-mail field and the Subscribe button. Please advise.

I also came accross this issue and found out that it needed hard coded changes.
The solution I found finaly was to restrict subscription to registered users only, and in that case - you can redirect the simplenews subscription to the site registry page.
Not perfect - but works if you can be sure people will subscribe to your site in order to receive newsletters...
