sorry I have real problems to follow your csv example.
for me, a comma separated import (csv) has a certain format.
a header line with the field definitions the import sql-query will use
and the rowsets, recordsets itself.

i.e I want to use the multichoice question recordset only with the choice of one or multiple answers but a feature rich question body (html with fck-e and images) and maybe 15! Answers with checkboxes and a few thousands of questions...

further problems:
the design of the test node is not table based. If an answers text gets to long it will continue below the radio or checkbox and not in a line to the right of them. (The html coding shows not an extra css tag but only form-radio-button and so on - so css styling seems to be a problem with this)

I can not configure the results table - it seems to have a fixed width somewhere but I did not find a view to adopt.

It should be possible to define a fixed height for the question section to present all answers, i.e with a bottom line to the question section, in the same vertical distance.

same with the distance between the last question and the fwd/back buttons.

There has been an information about a video tutorial on quiz but I did not find it.

Thank you
