Is there some example module which is using functions from Hierarchical Select Small Hierarchy module?

Or some example code which shows me how to generate form with Hierarchy Select?

There is some documentation included in the module itself. Scroll through it, and you'll find it. It's intended for custom hierarchies (i.e. hardcoded, not using Taxonomy).
But yes, this needs to be documented much better. I'll use this issue for that.

An example on how to use HS in Forms API can be found in API.txt.
