Per field settings (instead of per content type)

Issue Summary

The title says it all.
What we need here is a new database column to store the settings, some logic to allow fallback to content type settings if the field settings are not present, and a good UI.

I am looking forward to 1) #719068: show if ANY of controller fields are selected (OR vs AND controllers) and 2) #360824: Option to negate trigger values
