Option to issue a warning prior to deleting not working properly

Although I had set this module to issue a warning before deleting a user it did not do so. One of my former users complained of his account being deleted without any warning and I can see in the logs that the day after I installed the module it chose to delete a large group of users without issuing one. It sent warnings to some users, though. I don't understand the pattern.
