You're probably correct that there's some path by which things aren't cleaned up and should be. Please provide the steps that demonstrate that.

Sorry, I think this was the result of quick note taking from an xls ages ago. I believe the issue here was that when other modules deleted files it wouldn't remove the associated field values for that file.
I'm moving this over to file entity as (I think?) it belongs there.
Going to just close this issue. I don't understand what the actual task or bug is here.
