Send to editor button missing

Hi,

I have attached a print screen picture to this report, to show you what I see. The send to editor button missing.
Other issue, how can i adjust the popup window size, the popup opens in a resonable size but not near to the size that the imce application uses. I have adjust the 760 to 900 in the imce_set_inline.js file but the popup window size is stil the same.
