"From" Information Missing in E-mail

I'm getting blank "from" information on my e-mail even using the cvs version as mentioned in the earlier bug report on this topic.

I am using Drupal 4.7.3 and the Drupal mailer, no 3rd party mailing applications. My e-mail address is set in the settings area and is functioning properly in other Drupal mail forms and notifications on the site. Please let me know if there's some way to address this from my end or whether this is a coding issue.
