I made the tests on the last .dev version (announced here: http://drupal.org/node/894032#comment-3408924), here the results:
Browsers tested: IE6-7-8, Opera, Firefox, Safari, Chrome
SO: Seven
The permission settings are:
Anonymous user: can Views Downloads.
Registered user: can Download without email submission.
The submission form have an extra "Name" field, setted as "Active" and "Mandatory".
All the tests are made with "Anonymous user" to confirm the correct behaviour of the module to block or allow download operation in different situations.
TEST WITH CACHE DISABLED:
Opera, Firefox, Safari, IE7, IE6:
The modules seems work good with only one exeption: the .pdf file are ALWAYS avaiable to dowload!!
TEST WITH CAHCE ENABLED (NORMAL setting):
Opera, Firefox, Safari, IE7, IE6:
The module doesn't works fine.
Anonymous users can't download file in "/downloads" page and the "submission email" page is correctly displayed.
But when email and Name are submitted, after redirect on "/downloads" page, the users still cannot download it!
Naturally the .pdf file... are ALWAYS avaiable!
A partial resolution could be "zipping" thos type of files!
IE8 (in both situations)
The situation with this browser is probably the same, but in OS Seven removing I8 cookie (to make good test) is an impossible task! After first submission, the browser still continue to grant download even though: I restart browser, remove cookie manually, remove all internet temporary files!
Finally the test for this browser must be considered "partial".

I tryed, using "Cache Exclude" module to prevent caching for pages:
/downloads
/download*
But the problem remain!
The only way is excluding cache at all and avoid .pdf file format.
There's possibility to fix this problem??

I just tested the exact steps that you gave and could not reproduce this problem at all. No problem with PDF files or the like, which sounds unlikely anyway, because the module does not do any mime type specific tests or the like.
Are you sure that you deleted your cookies when retrying? Because every time you provide an email address, email download stores/modifies a cookie on you client to prevent annoying re-entering. It has nothing to do with the browser's cache.
I even tested (in IE) disabling the cache. The problem was still not apparent.
Maybe it is fixed in the dev version. Please download and test the dev version. Maybe hadsie fixed it there and that is why I am not seeing it.
I tested this on windows 7 using xampp.
Please answer to this so we can either close or fix this issue.

I hope to have a complete test before 15 Jan (I have to build up a test site ad re-add the module, uninstalled at the moment!).

geek.de.nz commentedOK. But by then I might not have much time for this issue. It would really help if you could test it today, so I can fix it tomorrow if need be. I attach the current dev snapshot for you to download. You can just unzip this folder in sites/all/modules and install the module on a fresh Drupal 6.20 install. Should only take half an hour.

Dret commentedI made a test on test-site (a copy of production site).
I have:
1) Disinstalled previously email-download module (even database tables).
2) Installed the new version attached in your last post.
3) Maden a series of tests with cache enabled/disable on Firefox, Chrome and IE8.
Results:
With cache enabled (1 hour):
Module doesen't works on any browser:
downloads is always avaiable (no redirect to email form page) but (especially on Firefox and Chrome) the file is opened as BIN on the browser itself (no prompt to SAVE or OPEN).Sometimes browser crash during download.
With cache disabled:
module work fine on FIrefox, Chrome BUT not in IE8 where download is avaiable (no redirect to form page) but an error message appear on page (not found page by apache).
Situation seems to be the same as last test.

Consider I'm using another file to make sme folder private (Webform Private Upload) and I have no problem.
If I should have enough time I'll try a second test later with a fresh Drupal installation.

Dret commentedI forgot: i'm using also "Javascript aggregator" and "Css Gzip" modules... (disableb and enabled in tests reported above with cache)

Problem seems to be related to:
Standard Settings (less problably)
-Cache
-Css Compression
-Javascript compression
Other modules (more problably)
-CSS GZIP
-Javascript aggregator
I test on another machine with Xp OS and IE8, result is:
-Works good with none of this settings/modules enabled
-Doesen't works (ask for email but fail to download) if all are enabled.
The behaviuor is different from the test site installation, but fail anyway.

If you do not want to give me details like this, you might want to make a setup where you can reliably reproduce it and give me the login details for admin.
