If you are able to perform two actions on an item such as "add note" and "check in", only one gets displayed.

I'll look into this in more detail later but haven't found anything obvious yet. This might be made more difficult due to library_links_display_unavailable and similar variables not being updated correctly.

Sadly, I think this may have been an issue for a while, but is only being caught now. I don't think it was related to the upgrade. Patch attached. Please let me know if this fixes it for you. If it does, I'll apply to the 6.x-2 branch.
