Problem/Motivation

There are errors reported when adding a payment method during checkout when using recent version of Drupal Commerce.

Proposed resolution

Update the PaymentMethodAddForm to take advantage of the base form that has been added to Commerce Payment.

API changes

If there are forms extending in custom modules extending the add payment method form, they should be updated accordingly.
We will be requiring Commerce 2.14 as minimum version where the most recent relevant changes were made.
