This module depends on main newsletter module and its menu is shown as last menu ( Export Subscribers )
User will be able to select which fields to be exported ( check box) , Subscribers list Options, Date format Selection ( Subscription date) , A checkbox if user wants first row as header in CSV .( PFA )

I have reviewed the functionality and seems to work fine. Maybe I would suggest allowing to add the field names as headers in the exported file and support for tab delimiter.
Please add documentation in your module page.
IMO this sounds more like a feature to improve newsletter module than a separated module, have you tried contacting newsletter mainteners?
