I just wrote one-liners and rolled a patch. I used new text from D7 when it existed and seemed appropriate.

Tihs is a duplicate of #576082: Missing doc for a few form.inc functions, and form.inc not up to doc standards, and it needs to be fixed in D7 before porting the fix to D6.
