No load contact form!!!

Hello.

This is a very easy query, I think. I guess I still do not quite understand the English language.

I turn on all the configuration features

I just want to load the contact form on an overlay

I could not make it work!

I've been using the following link:

And not work, the overlay showme "Request unsuccessful: Not Found"

Please help me
