I have some attachments coming in and they are being saved in 'sites/default/files/mailhandler_temp/'
But they don't seem to be clearing.
Should they be clearing from this folder 'sites/default/files/mailhandler_temp/' ?

The file is saved in that directory and added to the file_managed table with 'temporary' status. This means that Drupal should automatically delete the file after DRUPAL_MAXIMUM_TEMP_FILE_AGE has elapsed (default of 6 hours or so).

Can you confirm that DRUPAL_MAXIMUM_TEMP_FILE_AGE has elapsed, you've run cron, and the temporary files are still there?
