When I applied the block_cache.patch I got the folling rejection for hunk 4 and hunk 5.
Do I need to apply these by hand?

It's been re-rolled in the DRUPAL-5 version of advcache. It applies to DRUPAL-5-7 now. The DRUPAL-5-8 version still needs work. Check out the development branch of the advcache module to get the patch for DRUPAL-5-7.
