Three small amendments that are all related to using the new forms API.

This has been working for me for a few days now on a 4.7 beta 5 site without any apparent problems.

Duplicate of this issue. DId you not check the (small!) issue queue for this module before patching/posting?

Anyway, feel free to review/test my patch, and amend as you see fit. From a quick glance over the two patches, they're pretty much identical.
