I have a problem already mentioned by others on the menu icon that is not displayed. I have found that the cause seems to be a problem in the CSS path but despite the patches that are present here and I applied for some it still does not work.
Several questions:
Is it normal that it does not have to be "upload" button in the "Upload a new icon image"?
It seems that the CSS is taken into account in sites /default/files/css/menu_icons.css (see attached files).
Is there a patch or a solution to this problem.

Indeed the problem seems to be the path that is created in sites /default/files/css/menu_icons.css
When your Drupal site is not installed on the root of the website.
I tested changing the file you mention to manually add at the beginning the /[DIRECTORY]/ of the installation and it worked. BUT the css files is recreated every time you change the position of the block, so after a few tries it reverted to not working.
There seem to be a patch going around for the module, but I haven't found it yet. I found one, but it destroyed the code and had to revert to the original version.
