The TOC works great but one thing that I've noticed is that the layout doesn't seem consistent. Some items are directly next to the number while others appear on the line below it. (see the attached image).

Never mind. It turns out that I had some line breaks in the html code that were carrying over to the TOC. I removed the breaks and everything cleared up.

That's rather strange - the br tag wasn't being stripped? Titles are passed through strip_tags so they shouldn't make a difference.

It wasn't a br tag, it was an enter break, as in I had hit enter on the keyboard. It seemed odd to me also but as soon as I removed the enter breaks from the content the TOC formatted perfectly.
