Fixing http://drupal.org/node/248297 caused by menu module api update.

Fixing http://drupal.org/node/248267 which was caused by the API change in menu module.
Also some documentation updates.

fixed an access permission problem for anonymous users who cannot access content.
fixes a few other minor code style issues, etc.

This release of Fivestar adds several new features and fixes a few outstanding issues. Note that this release of Fivestar has significantly changed the theme_fivestar_summary() function. Any sites that have over-ridden this theme function should take care when upgrading.

The Drupal 6 1.12 final version is feature equivalent to the 1.12 beta 1 version except CCK implementation is now completed. The Drupal 5 and 6 versions of Fivestar are now completely equal in feature implementation.
